Summary:
Seeking a proactive and compassionate Medical Assistant to support a clinical team and patients, focusing on patient engagement, care coordination, and administrative duties within a heart failure and value-based care program.
Responsibilities:
- Engage with patients in waiting areas to explain clinical studies and facilitate enrollment in care management programs.
- Coordinate patient care activities, including post-discharge follow-up, telehealth scheduling, and remote patient monitoring.
- Conduct patient intake via phone, gathering information on symptoms, vital signs, and social determinants of health.
- Maintain and manage a patient cohort requiring frequent follow-up, care management, and coordination with clinical teams.
- Ensure accurate and confidential maintenance of patient records, including lab results, insurance information, and prescription refills.
- Deliver patient education and information to patients and caregivers, supporting ongoing health care needs.
- Perform clerical and triage duties, process communications, and document work processes according to protocols.
- Participate in interdisciplinary team meetings and collaborate with staff to optimize patient outcomes and meet performance metrics.
Qualifications:
- Certified Medical Assistant through a recognized national certification exam (e.g., AAMA or NCCT) within one year of issuance.
- High school diploma or GED required.
- At least 3 months’ experience in a medical office setting preferred.
- Strong computer skills and proficiency with electronic health records (e.g., Athena, eCW, NextGen), Microsoft Word, and Excel preferred.
- Knowledge of medical terminology, medication procedures, and basic CPT & ICD-10 coding.
- Excellent organizational, interpersonal, and communication skills; ability to multi-task and prioritize duties efficiently.
- Ability to remain calm and effective in a busy environment, respond appropriately to urgent situations, and adhere to safety and attendance standards.
- Experience with telehealth and cardiology is a plus, but not required.
